Readmission to a hospital can cause dire trauma for a senior, so much so that the Centers for Medicare and Medicaid Services (CMS) have just announced the expansion of their Hospital Readmission Reduction Program (HRRP) to include skilled nursing facilities.
As of October 1, 2018, CMS will now withhold 2% of a skilled nursing facility’s Medicare reimbursement until they have shown that they can keep down the number of residents who return to a hospital within 30 days of discharge. According to America’s Health Rankings*, 14.9% of Medicare enrolled Americans age 65 and older were readmitted to the hospital within 30 days of discharge. Spring Hills Home Care Services’, a JCAHO-accredited home care agency, client rehospitalization rate is only 2.1% due to core services they provide.
Andre Gomez, Vice President of Home Care Operations at Spring Hills Home Care, said, “We see the severe consequences of frequent rehospitalization for our clients. To avoid this, we have identified the top five risk factors that lead to readmission, and we have worked these into the standards of care at Spring Hills Home Care.” These risk factors are:
- Lower Extremity Weakness
- Medication Errors
- Nutritional Insufficiencies
- Social Isolation
- Transfer Trauma
Spring Hills Home Care takes pride in the relationship they have developed with skilled nursing facilities to alleviate the need for rehospitalization. Gomez added, “Our team works collaboratively with staff at skilled nursing facilities to ease the recovery process and reduce hospital readmission.”
